**Each family will only need to make one login for the whole family!**

• Select your State: Louisiana
• Select your institution: Click LA 4-H Youth Development
• Select your County: Lincoln

Enter info in all required fields. Use your PARENT OR GUADIAN'S email and phone Click "create account".

Enter your mailing address and click "verify".

• Which program would you like to join? Click "Louisiana 4-H " then "next".
• Enter personal and demographic info in all required fields. Info should be for the youth being enrolled. Click "next" after each screen.
• How would you like to participate? Click "I want to join 4-H as a new or returning youth" then "finish".
• Select Clubs: Find your school club. Your school club is whatever school your child is attending for the school year. If you are homeschooled, choose homeschool. If you are attending a school outside of Lincoln Parish but you LIVE inside of Lincoln Parish, you will choose Member at Large.
• Select Project: You may choose multiple. You can choose any of them that you like. It will not mean your child will be forced to only participate in those projects. Your child will be able to participate in all projects that Lincoln Parish offers regardless of what they choose here.
• Years in project: This number does not matter.
• Questions: Enter info in all required fields.

For the "school" heading:

• If you attend a public school:

1. Click the first circle
2. County & District: "Lincoln"
3. Select your school

• If you are homeschooled or member at large:

1. Click the second circle
2. Fill in the school’s name and type

Health Form:

Answer health questions and read medical release.

Click "agree" and enter required info.

Consents:

Includes Hold Harmless, Media Release, and Code of Conduct.

Read through each then click "agree" and enter required info.

To submit enrollment, you must choose “Pay By Check“ or pay by card, select “I understand”, enter info, and click “Confirm”! After confirming click “Next” and then click the green “Submit” button. Enrollment is not complete until you have clicked the green “Submit” button. This will bring you back to your family page and “Enrollment Submitted” will be listed by your member’s name.

To enroll another member, follow the instructions above for returning members or select “Add Member” to start enrollment for a new family member. Do not create a new account for a member in the same home.
